About the Role

We are seeking a highly skilled and motivated Social Worker to join our Adult Social Care team at Bracknell Forest Council. As a Social Worker, you will play a vital role in supporting vulnerable adults and their families to live independently and safely in their own communities.

Key Responsibilities
  • Conduct comprehensive assessments to identify the needs of service users and develop personalized care plans.
  • Work collaboratively with multidisciplinary teams, including health and social care professionals, to ensure seamless service delivery.
  • Provide support and guidance to service users and their families to empower them to make informed decisions about their care.
  • Participate in safeguarding assessments and planning to ensure the safety and well-being of vulnerable adults.
  • Act as a key worker for service users, providing ongoing support and advocacy to ensure their needs are met.
Person Specification
  • Recognized Social Work qualification and registration with Social Work England.
  • Safeguarding level 2 (or equivalent).
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions.
  • Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of the service.
