About the Role:
We are seeking a highly skilled and experienced Social Worker/Approved Mental Health Professional to join our Community Mental Health Team in Bracknell Forest. As a care coordinator, you will work closely with individuals, families, and communities to provide person-centered and outcome-focused support.
Key Responsibilities:
- Work as an AMHP in a CMHT with a caseload and participate in a locality duty rota.
- Complete planned assessments and contribute to service delivery that is person-centered and outcome-focused.
- Ensure collaborative work with colleagues and other professionals and agencies to provide a multi-agency team of support.
Requirements:
To be considered for this role, you must have a Social Work qualification (Diploma in Social Work, Social Work Degree, or equivalent) and at least 3 years of post-qualified experience working as a Qualified Social Worker in a similar team.
