We are seeking a highly skilled and experienced Senior Clinical Assessor to join our Hampshire and Isle of Wight ICB (104) team. As a Senior Clinical Assessor, you will play a crucial role in ensuring that individuals receive the necessary care and support to meet their needs.
Key Responsibilities- Participate in complex information analysis and review of enhanced care packages to ensure ongoing requirements for clients entitled to S117 aftercare.
- Work with individuals who have a diagnosis of a mental health condition and/or Autism, providing support and guidance to ensure they receive the necessary care and support.
- Develop and implement care plans that meet the individual's needs, working closely with healthcare professionals and other stakeholders.
- Conduct annual reviews and right-size longstanding packages of care to ensure they are meeting the individual's needs.
- Support the Multi-Disciplinary Team (MDT) in screening and approving new funding applications, attending discharge planning meetings, and providing proactive case management to complex and rehabilitation clients.
- Registered Health or Social Care Professional with current registration with a professional body (NMC, HCPC, or SWE).
- Educated to degree level in a relevant subject or equivalent level of experience working in health or social care.
- Experience of assessment of health and social care needs, care planning, and coordination.
- Knowledge and experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues.
- Ability to work effectively under stressful conditions in a rapidly changing environment.
- Good knowledge of Mental Capacity Act and relevant legislation.
- Effective working knowledge of Microsoft software.
