About the Role

We are seeking a highly skilled and experienced Senior Clinical Assessor to join our Hampshire and Isle of Wight ICB (104) team. As a Senior Clinical Assessor, you will play a crucial role in assessing the healthcare needs of individuals with mental health conditions and acquired brain injuries, ensuring they receive the appropriate care and support.

Key Responsibilities
  • Conduct comprehensive assessments of individuals' healthcare needs, including mental health and ABI.
  • Develop and implement personalized care plans, ensuring timely and effective delivery of care.
  • Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and carers, to ensure seamless care coordination.
  • Provide expert advice and guidance to individuals, families, and carers on healthcare options and support.
  • Contribute to the development and implementation of healthcare policies and procedures, ensuring compliance with relevant legislation and guidelines.
Requirements
  • Registered Health or Social Care Professional with current registration with a professional body (NMC, HCPC, or SWE).
  • Experience in assessing healthcare needs, care planning, and coordination.
  • Knowledge and experience working with individuals with mental health conditions and/or ABI.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work autonomously, with strong problem-solving and analytical skills.
