Senior Clinical Assessor

About the Role
As a Senior Clinical Assessor within the Hampshire and Isle of Wight ICB (104), you will play a vital role in enabling timely provision of appropriate care and support, ensuring that people's needs are effectively met while promoting Personal Health Budgets to encourage independence.
Main Responsibilities
- Screen and approve funding applications for clients who require packages of care or placements that are eligible for S117 aftercare entitlement under the Mental Health Act 1983.
- Work closely with Southern Health Care Coordinators and social services colleagues to ensure commissioned care is safe, appropriate, and cost-effective through ongoing review and assessment.
- Conduct standard annual reviews and right-size longstanding packages of care.
- Support the MDT to screen and approve new funding applications.
- Attend discharge planning meetings, CPAs, CTRs.
- Support and manage complex duty cases.
- Provide proactive case management to individual complex and rehabilitation clients.
- Facilitate timely discharges from acute hospitals and placements where clients are no longer gaining benefit from that service.
- Support with safeguarding.
- Support with setting up of PHBs.
Requirements
- Registered Health or Social Care Professional with current registration with professional body NMC, HCPC or SWE, including AMHP's and mental health social workers or significant relevant experience.
- Experience of assessment of health and social care needs – care planning and coordination.
- Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues.
- Ability to work effectively under stressful conditions in a rapidly changing environment.
- Good knowledge of Mental Capacity Act and relevant legislation.
- Effective working knowledge of Microsoft software.
- A good understanding of the health and social care environment and roles and responsibilities within it.
