Key Responsibilities- Conduct comprehensive assessments of individuals' healthcare needs, including mental health and ABI.
- Develop and implement personalized care plans, ensuring timely and effective delivery of care.
- Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and carers, to ensure seamless care coordination.
- Provide expert advice and guidance to individuals, families, and carers on healthcare options and support.
- Contribute to the development and implementation of healthcare policies and procedures, ensuring compliance with relevant legislation and guidelines.
- Registered Health or Social Care Professional with current registration with a professional body (NMC, HCPC, or SWE).
- Experience in assessing healthcare needs, care planning, and coordination.
- Knowledge and experience working with individuals with mental health conditions and/or acquired brain injuries.
- Excellent communication, interpersonal, and presentation skills.
- Ability to work effectively under stressful conditions, prioritizing tasks and managing multiple responsibilities.
