Medicines Optimisation Expert

King's College Hospital NHS Foundation Trust is seeking a highly skilled Pharmacist Specialist to join our team. As a key member of our multidisciplinary team, you will play a crucial role in delivering high-quality, cost-effective, patient-focused pharmaceutical care to patients.
Main Responsibilities- Medicines Optimisation: Work with the general practice team to provide specialist advice to patients and undertake full patient-centred medication reviews to proactively manage people with complex polypharmacy, particularly the elderly and those with multiple conditions.
- Interface and Discharge Liaison: Provide support to the wards and units within the Care Group, ensuring seamless communication and coordination of care.
- Policy and Service Development: Propose and implement policies, guidelines, and procedures to improve pharmacy services, ensuring compliance with local and national policies.
- Staff Management and Training: Participate in the recruitment and selection of pharmacy students and staff, and assist in training and assessment of junior pharmacy staff.
- Research and Development: Initiate or contribute to relevant practice research or audit projects, and participate in clinical audit in medicines optimisation initiatives.
- Registered Pharmacist with the General Pharmaceutical Council.
- Postgraduate Certificate in Pharmacy Practice or equivalent.
