About the Role

Lancashire & South Cumbria NHS Foundation Trust is seeking a highly skilled and dedicated Liaison and Diversion Practitioner to join our team. As a Liaison and Diversion Practitioner, you will play a vital role in identifying individuals with complex needs and vulnerabilities within the criminal justice system, providing them with the necessary support and interventions to address their needs and reduce the risk of reoffending.

Key Responsibilities

  • Work closely with police custody suites, courts, and other agencies to identify individuals with complex needs and vulnerabilities
  • Conduct initial assessments and provide interventions to address mental health, substance misuse, and learning disability needs
  • Develop and implement individualized care plans to support individuals in achieving their goals and reducing the risk of reoffending
  • Collaborate with other agencies to provide a seamless service and ensure that individuals receive the necessary support and interventions
  • Provide training and education to other agencies and professionals on Liaison and Diversion services and best practices

Requirements

  • Degree qualification in a relevant subject or equivalent experience
  • Knowledge of mental health issues, substance misuse, and learning disability
  • Experience of working with individuals with complex needs and vulnerabilities
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
