About the Role

We are seeking a highly skilled and compassionate Liaison and Diversion Practitioner to join our team at Lancashire & South Cumbria NHS Foundation Trust. As a key member of our Liaison and Diversion Service, you will play a vital role in identifying and supporting individuals with complex needs within the criminal justice system.

Key Responsibilities
  • Conduct initial assessments of health and social needs to identify vulnerabilities and unmet needs
  • Develop and implement personalized support plans to address these needs and promote positive outcomes
  • Liaise with healthcare and social care professionals to ensure seamless transitions and effective interventions
  • Work collaboratively with police and court staff to provide timely and relevant information to inform decision-making
Requirements
  • Minimum of 2 years' experience working with individuals with complex needs
  • Relevant qualifications in health and social care, such as NVQ Level 3 or Diploma in Health and Social Care
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
