Job Overview

We are seeking a highly skilled and compassionate Community Psychiatric Nurse to join our team at West London NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing specialist assessments, reviews, and nursing interventions to individuals with cognitive impairment and dementia.

Main Responsibilities
  • Conduct comprehensive assessments of individuals and their families to identify their needs and develop personalized care plans.
  • Work collaboratively with other healthcare professionals to ensure seamless care coordination and effective risk management.
  • Provide education and support to individuals, families, and carers to promote self-management and empowerment.
  • Develop and implement individualized care plans that address the physical, social, and psychological needs of individuals with cognitive impairment and dementia.
  • Collaborate with other healthcare professionals to ensure that individuals receive comprehensive and coordinated care.
Requirements
  • Professionally qualified and registered Nurse with current NMC registration in Mental Health/LD/Adult Nursing.
  • Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
  • Experience of supervising staff and managing clinical caseloads.
  • Ability to work within a multidisciplinary community team and demonstrate a commitment to the provision of high-quality and safe services.
