We are seeking a highly skilled and experienced Band 6 Community Psychiatric Nurse to join our team at Service Care Solutions Ltd. As a key member of our Memory Assessment Services team, you will be responsible for providing high-quality care to patients with dementia and their families.
- Develop and implement effective care plans for patients with dementia, working closely with multidisciplinary teams to ensure seamless care delivery.
- Conduct comprehensive assessments of patients' needs, identifying risk factors and developing strategies to mitigate them.
- Provide expert guidance and support to other staff members, promoting a culture of excellence and continuous learning.
- Collaborate with GPs and other services to ensure the highest possible standards of patient care.
- Participate in mentoring and clinical supervision, promoting the Trust's values and adhering to the NMC and HPC code of conduct.
- Hold a valid PIN number as a registered mental health nurse.
