Key Responsibilities:
  • Develop and implement effective care plans for patients with dementia, working closely with multidisciplinary teams to ensure seamless care delivery.
  • Conduct comprehensive assessments of patients' needs, identifying risk factors and developing strategies to mitigate them.
  • Provide expert guidance and support to other staff members, promoting a culture of excellence and continuous learning.
  • Collaborate with GPs and other services to ensure the highest possible standards of patient care.
  • Participate in mentoring and clinical supervision, contributing to the development of junior staff members.
Requirements:
  • Hold a valid PIN number as a registered mental health nurse.
  • Fully enhanced DBS check.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
