Job Title: Adults Team Leader - Community Services

We are seeking an experienced and skilled Adults Team Leader to join our Community Services team at Navigo Health and Social Care CIC.

About the Role:

The successful candidate will be responsible for leading a team of occupational therapists in providing high-quality care to adults in the community. This will involve:

  • Developing and implementing care plans that meet the individual needs of service users
  • Leading and managing a team of occupational therapists to ensure effective delivery of care
  • Collaborating with other healthcare professionals to ensure seamless care pathways
  • Conducting comprehensive assessments of service users' occupational performance areas
  • Developing and delivering training to staff and students on occupational therapy practices
Key Responsibilities:

The successful candidate will be responsible for:

  • Planning and coordinating OT care packages for individuals and groups
  • Conducting assessments and analyzing service users' occupational performance areas
  • Developing and implementing individualized interventions to maximize service users' skills and function
  • Collaborating with service users and carers to identify OT goals and develop care plans
  • Participating in caseload management and ensuring relevant information is available
Requirements:

To be successful in this role, you will need:

  • Significant experience in occupational therapy, preferably in a community setting
  • Strong leadership and management skills, with the ability to motivate and develop a team
  • Excellent communication and interpersonal skills, with the ability to work effectively with service users, carers, and other healthcare professionals
  • Ability to analyze complex information and make informed decisions
  • Knowledge of occupational therapy practices and principles, with a focus on community-based care
