We are seeking a highly skilled and experienced Senior Palliative Care Nurse to join our Night Time Community Palliative Care Service. The successful candidate will be responsible for providing assessment, care, and implementing palliative treatment plans for patients at the end of life or in the deteriorating or unstable phase of their illness.
Key Responsibilities- Provide holistic assessment and interventions such as pain and symptom control, management of subcutaneous syringe pumps, re-hydration, emergency unblocking catheters/catheterisation, supporting management of urgent nursing related issues, intravenous therapy etc.
- Recognise and avoid situations that may be detrimental to the health and wellbeing of individuals and escalate where necessary.
- Provide support and guidance to staff in the monitoring planning, delivery and evaluation of individualised care.
- Be responsible for the correct administration of prescribed medication including management of syringe pumps, enteral nutrition, intravenous therapy etc.
- Act as an advocate for patients, to ensure a patient centred approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
- Ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, culture, social circumstances, religious beliefs and doctrines.
- NMC Registered Adult Nurse, band 5 or above
- Previous Experience working with people who have life limiting conditions who are nearing the end of their lives
- Experience of nursing patients with indwelling catheters, enteral nutrition, etc
- Experience of teaching/assessing/ supervising others in a clinical setting
- Supervision and support of junior staff
The Night Time Community Palliative Care Service operates between the hours of 20:00 and 08:00 - 7 days per week. The successful candidate will be required to work collaboratively with multi-disciplinary Specialist Palliative Care and Community End of Life teams and @Home service to meet the needs of patients referred to the service.
The service is an innovative response community based night service offering nursing support to patients with palliative care needs in their own home within the Boroughs of Lambeth and Southwark. The team supports patients overnight with uncontrolled symptoms, personal care, telephone advice, patient/carer distress, verifying expected deaths and performing care after death.
The successful candidate will be responsible for mentoring and supporting assigned nurses and nursing assistants to deliver high quality, safe and effective care on a day-to-day basis. They will also participate in rotation of coordination of shifts and work collaboratively with the Senior Nurse Practitioner to support a group of patients in collaboration with the multidisciplinary team, delivering individualised and personalised direct patient care overnight.
The successful candidate will be required to demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care. They will also be responsible for ensuring that staff take precautions to minimise the risk of cross infection in relation to the safe disposal of equipment and body fluids (including blood).
The successful candidate will be required to promote high quality clinical care, assist in the development of the service through the use of evidence based practice and clinical standards.
