Brook Street Social Care is seeking a skilled Complex Care Manager to join our team. As a Complex Care Manager, you will be responsible for coordinating and delivering complex home care services to individuals with medium to long-term care needs.
Key Responsibilities:- Coordinate services for new clients, ensuring high standards of care are maintained.
- Maintain constant contact with service users to ensure person-centred care.
- Liaise with clinicians, Clinical Commissioning Groups (CCGs), and other professional bodies to ensure care packages meet defined clinical standards.
- Prepare and send monthly reports to the CCG on ongoing care provided.
- Provide high-quality care in the field, covering emergency shifts if needed.
- Valid driving licence and own vehicle.
- At least 5 years of experience in community care.
- Ideally, experience in complex care areas such as ventilation and spinal injuries.
- Ability to work independently and remotely.
- Willingness to visit the office bi-weekly for management meetings.
- Passion for supporting service users, families, and staff members.
- Proven ability to work with families and individuals facing significant life changes.
