About the Role

We are seeking a skilled Mental Health Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our multidisciplinary team, you will provide high-quality mental health services to individuals under the Enhanced Care Pathway.

Key Responsibilities

  • Develop and maintain effective relationships with service users, working collaboratively with them and their carers to ensure the best possible outcomes.
  • Provide screening and triage assessments for allocated service users, identifying their needs and developing personalized care plans.
  • Deliver a range of mental health interventions, including assessment, treatment, and care planning, in accordance with best practice guidelines.
  • Work closely with senior members of the multidisciplinary team to coordinate care for designated service users, ensuring seamless transitions and effective communication.
  • Contribute to the evaluation of care, using data and audit processes to inform service improvements and ensure high-quality care.
  • Participate in regular team meetings, peer supervision, and training to maintain professional development and expertise.

Person Specification

  • Relevant education to degree level or equivalent experience in mental health or a related field.
  • Experience working with service users and carers, with a focus on developing therapeutically effective relationships.
  • Ability to form effective working relationships with other partners, patient groups, and stakeholders.
  • Organizational and time management skills, with the ability to work in an ever-changing environment.
