The National Health Service (NHS) has developed various sub-acute services in recent years, guided by NHS white papers such as the NHS Long Term Plan and the NHS Five Year Forward View. These documents recommend that NHS trusts deliver more care and services closer to patients' homes rather than in acute hospital settings.

As the demand on emergency departments across the UK increases, innovative care models will be essential in alleviating some of these pressures by redirecting work from hospitals to community settings through initiatives like the 2-hour Urgent Community Response and virtual ward services.

In Cheshire West, the 'Hospital at Home' initiative has successfully implemented virtual wards, enabling patients to receive necessary care at home safely and conveniently, rather than being admitted to a hospital. The NHS is increasingly adopting virtual wards to support individuals in their own homes, including care facilities.

The Hospital at Home workforce aligns well with the NHS model, providing hospital-level care at home. Our team includes experienced General Practitioners, advanced clinical practitioners with prescribing authority, registered therapists/nurses, and support staff, offering a comprehensive 7-day service from 8 am to 8 pm. This approach ensures wrap-around care and an exceptional patient experience, as evidenced by monthly feedback from patients.

Main Responsibilities

We are seeking dedicated, enthusiastic, and highly skilled Registered Nurses to join our Hospital at Home team, supporting this innovative approach to healthcare delivery.

We welcome applications from Registered Nurses with experience in acute medicine as well as community care, particularly in managing the long-term health needs of our aging population.

The primary focus of this role is to deliver patient-centered clinical care, which includes assessing and diagnosing patients and providing ongoing acute management under the supervision of General Practitioners and Advanced Clinical Practitioners within the Hospital at Home team. These positions aim to enhance patient outcomes by providing safe, compassionate, and effective care.
