About the Role

We are seeking a highly motivated and experienced Senior Social Worker to join our Transitions Team in Hammersmith & Fulham Council. The successful candidate will be responsible for supporting young people as they transition through our Children's Services into Adulthood.

Key Responsibilities
  • Work with young people and their families to effectively plan for adulthood, ensuring they are supported to navigate the complex route into Adult Services.
  • Develop and implement creative, community-based plans to address the needs of young people, including those with Special Educational Needs and Disabilities.
  • Liaise with a wide range of professionals, including more senior levels, to ensure effective multi-agency collaboration.
  • Assess and manage higher levels of risk, striking a balance between support and control.
Requirements
  • Recognised social work qualification (DipSW, CQSW, CSS or approved equivalent, BA Hons Social Work or other relevant degree in Social Work).
  • Social Work England registration.
