About the Role

Livewell Southwest CIC is seeking a highly skilled and experienced Registered Mental Health Nurse to join our team as a Senior Clinical Nurse Specialist. This is a senior clinical role responsible for ensuring clinical effectiveness and coordinating services for individuals with complex learning disabilities.

Key Responsibilities
  • Clinical Responsibilities
    • Participate in the undertaking of health needs reviews and health assessments to identify needs and plan care accordingly.
    • Identify, plan, and manage risk of an individual's care needs, relating to physical health, behavioral, and mental health.
    • Be responsible for assessing, planning, and making recommendations for future care in partnership with patients, carers, and multi-professional agencies, including case conferences for reviewing packages of care and care coordinating as appropriate.
    • Liaise with Primary and Secondary care agencies to ensure better health outcomes for individuals.
    • Maintain effective and appropriate communication networks with members of the multi-disciplinary team, the independent sector, and all stakeholder agencies and organizations to ensure that individual care needs can be met.
    • Provide evidence-based advice and guidance to care staff on the management of health needs to patients and signpost to other services.
    • Undertake health promotion/education whenever possible to all adults with LD and contribute to planned health promotion activities.
    • Work with all other agencies with regards to the safeguarding process.
  • Managerial Responsibilities
    • Line Management responsibilities for staff Band 5 and below.
    • Be responsible for coordinating reviews as per guidelines.
    • Prioritize caseload to meet competing demands.
    • Be aware of and abide by the LWSW policies, standards, and Clinical Governance initiatives.
    • Present confidentiality and be aware of the Data Protection Act, access to health records, and consent for assessment process.
    • Involvement in continual audit and review of services to determine quality.
    • Maintain safe record-keeping and confidential practice, in line with LWSW and professional guidance, and be aware of the Data Protection Act, access to health records, and consent for assessment process.
    • Ensure electronic record-keeping policy is adhered to.
  • Professional Responsibilities
    • Maintain a high professional standard of conduct at all times in accordance with NMC/professional bodies code of professional conduct. Maintain an active status on NMC/professional bodies register.
    • Assume responsibility for own professional development and personal knowledge.
    • Monitor own performance against agreed objectives through the process of appraisal and personal development plans and professional body regulations.
    • Maintain professional expertise by arranging and attending meetings, study days, and mandatory training and service training.
    • Plan and participate in the supervision and provision of experience for all nurse learners and other personnel as appropriate.
    • Participate in regular line management, caseload management, and clinical supervision.
    • Deliver appropriate training and subsequent support to other staff, students, and outside agencies when required.
Requirements
  • DipHE Nursing and/or Degree.
  • Professional Registration.
  • Evidence of continuing professional development.
  • Leadership qualification/knowledge.
  • Knowledge of Continuing Health Care (CHC) processes.
  • Knowledge of the Mental Health Act.
  • Able to demonstrate understanding and competency in assessment, care planning, and review.
  • A good understanding of the safeguarding adult process and relevant issues.
  • Able to problem solve and be creative in care planning.
  • Full driving license is required as the post holder will be traveling out of area.
  • The ability to be away from home overnight is an integral part of the role.
