Job Summary

The Nelson Trust is seeking a skilled Recovery Support Specialist - Case Manager to join our team. As a key member of our residential addiction treatment team, you will play a vital role in supporting clients in their recovery journey.

Key Responsibilities

  • Develop and implement individualized care plans to meet the unique needs of each client
  • Provide one-on-one support and guidance to clients in a residential setting
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support
  • Conduct regular assessments and evaluations to monitor client progress
  • Maintain accurate and up-to-date records of client interactions and progress

Requirements

  • Experience working in a residential addiction treatment setting
  • Strong understanding of addiction and recovery principles
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a variety of shifts, including weekends and bank holidays
