An exciting opportunity has emerged for a dedicated and passionate fitness program coordinator to join the South West Yorkshire Partnership NHS Trust within the cardiac and pulmonary rehabilitation team. This community-focused group is committed to delivering exceptional care and support for individuals participating in cardiac and pulmonary rehabilitation initiatives. The role will primarily be based in a designated center, with responsibilities that require travel within the local area to provide services across various sites.

Key Responsibilities

The successful candidate will collaborate within a team while also managing individual responsibilities. You will be tasked with overseeing your own caseload and contributing to the daily management of group activities. Key duties include:

  • Monitoring patients during exercise sessions.
  • Designing and advancing exercise programs.
  • Conducting educational workshops.
  • Documenting electronic patient records.

Experience in facilitating exercise components of cardiac or pulmonary rehabilitation and working in a community environment is preferred. A solid understanding of cardiac and pulmonary conditions, such as heart disease and COPD, will be advantageous.

Professional Development

There will be a strong emphasis on training and professional growth. The selected candidate will receive regular supervision and will be encouraged to pursue training opportunities to enhance their continuous professional development (CPD).

Job Responsibilities

The primary responsibilities of this role include:

  • Planning, organizing, executing, and evaluating exercise and educational sessions for patients with cardiac or pulmonary conditions in a community setting.
  • Assessing the suitability of patients for group activities and making necessary adjustments.
  • Managing a personal caseload of individual patients with cardiac or pulmonary conditions.
  • Maintaining accurate treatment records and ensuring data is entered into relevant IT systems.
  • Developing and delivering specialized therapeutic exercise groups.
  • Conducting educational sessions related to the clinical area.
  • Leading and demonstrating activities to new or less experienced staff, students, or volunteers.
  • Participating in audits and surveys as needed and contributing to research and development projects.
