About the Role

We are seeking a highly skilled and dedicated Care Coordinator to join our team at Weston Favell Primary Care Centre - Favell Plus GP Surgery. As a Care Coordinator, you will play a vital role in supporting patients and their families in our community.

Key Responsibilities
  • Coordinate and deliver weekly PCN-led MDT meetings, ensuring seamless communication and collaboration among team members.
  • Utilize population health intelligence and PCN/Partner data to proactively identify and work with patients to deliver personalized care.
  • Support patients in utilizing decision aids to prepare for shared decision-making conversations.
  • Holistically bring together patients' identified care and support needs, exploring options to meet these within a single personalized care and support plan (PCSP).
  • Provide coordination and navigation for patients and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals.
  • Work with GPs and other primary care professionals to identify and manage a caseload of patients, referring them back to other health professionals as necessary.
  • Raise awareness within the PCN of shared decision-making and decision support tools.
Person Specification
  • GCSE or equivalent grade C level qualification in Maths and English.
  • Minimum 1 year's experience of working with healthcare professionals and/or previous experience in the NHS or social care or relevant field.
  • Experience of coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes.
  • Experience providing advice/signposting to patients.
  • Experience of undertaking quality improvement activity.
  • Excellent organizational and administration skills.
  • Ability to analyze and interpret information and present results in a clear and concise manner.
  • Able to prioritize and manage own workload.
