Frailty Nurse Specialist

We are seeking an experienced and dedicated Frailty Nurse to join our clinical team on a part-time basis. The successful candidate will be responsible for providing high-quality healthcare to our frail population, working closely with our multidisciplinary team to deliver advanced nursing practice and services.
Main Responsibilities- Proactively manage patients living in care homes and those who are housebound, focusing on admission avoidance, improved diagnostic rates, and better coordination of care.
- Support the delivery of advanced nursing practice and services for older and vulnerable patients, including providing specialist nursing advice and support to patients, their families, and other healthcare professionals.
- Work as part of a practice clinical team to cover all patients' healthcare needs in a holistic way, promoting health and wellbeing, self-care, and independence.
- Provide proactive care to patients within care homes, including weekly visits and home visits as needed.
- Develop and implement admission avoidance plans, working with patients, post-discharge from hospital, to ensure follow-up arrangements are in place.
