Job Title: Frailty Nurse

We are seeking an experienced and dedicated Frailty Nurse to join our clinical team at Harvey Group Practice and Grange Street Surgery. As a key member of our team, you will play a vital role in providing high-quality healthcare to our frail population.

Job Summary

The successful candidate will be responsible for managing patients living in care homes and those who are housebound. This will involve proactive care, admission avoidance, improved diagnostic rates, better coordination of care, increased use of anticipatory care plans, reduction in falls, and improved management of chronic conditions.

Key Responsibilities
  • Support the delivery of advanced nursing practice and services for older and vulnerable patients.
  • Provide support and care to patients with long-term conditions or complex frailty needs.
  • Work as part of a practice clinical team to cover all patients' healthcare needs in a holistic way.
  • Provide proactive care to patients within care homes, including weekly visits and home visits during the week.
  • Provide specialist nursing advice and support to patients, their families, and other healthcare professionals regarding frailty and admission avoidance.
  • Work with patients post-discharge from hospital to ensure follow-up arrangements and admission avoidance plans are in place.
  • Promote health and wellbeing, self-care, and independence.
  • Attend clinical governance and other multi-disciplinary meetings as required.
  • Liaise and communicate effectively with the multidisciplinary and wider community teams across health and social care.

Person Specification

Essential Qualifications:

  • Current NMC registration.

Essential Experience:

  • Management and knowledge of long-term conditions.
  • Involvement in implementing and using protocols and clinical guidelines.
  • Experience of audit.
  • Management of long-term conditions.
  • Minor illness experience.

Desirable Qualifications:

  • Experience of working in a primary care setting.
  • Non-medical prescriber.
  • Post-qualification certificates relating to appropriate aspects of this role.
