Job Summary

We are seeking a highly skilled and experienced Frailty Nurse Specialist to join our clinical team on a part-time basis. The successful candidate will be responsible for providing high-quality healthcare to our frail population, working closely with our multidisciplinary team to deliver advanced nursing practice and services.

Main Responsibilities
  • Proactively manage patients living in care homes and those who are housebound, with a focus on admission avoidance, improved diagnostic rates, and better coordination of care.
  • Support the delivery of advanced nursing practice and services for older and vulnerable patients, including the management of long-term conditions and complex frailty needs.
  • Work collaboratively with our clinical team, including GPs, clinical pharmacists, and the wider nursing team, to provide holistic care to patients.
  • Provide proactive care to patients within care homes, including weekly visits and home visits as needed.
  • Offer specialist nursing advice and support to patients, their families, and other healthcare professionals regarding frailty and admission avoidance.
  • Work with patients post-discharge from hospital to ensure follow-up arrangements and admission avoidance plans are in place.
  • Promote health and wellbeing, self-care, and independence among our patients.
  • Attend clinical governance and other multidisciplinary meetings as required.
  • Liaise and communicate effectively with the multidisciplinary and wider community teams across health and social care.
