Job Summary

We are seeking a highly motivated and dynamic Diabetes Specialist Nurse to join our community team at Hertfordshire Community NHS Trust. The successful candidate will work as part of the wider Integrated Diabetes Service, providing clinics and group education sessions to adults with diabetes in South and West Hertfordshire.

Main Responsibilities

  • Deliver high-quality care to patients with diabetes, working closely with the multidisciplinary team.
  • Develop and implement education programs to support patients in managing their diabetes.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Contribute to the development and implementation of service improvement initiatives.

Requirements

  • Registered nurse with postgraduate experience in diabetes care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Good IT skills and ability to use electronic records.
  • Valid UK driving license and access to a car for business purposes.
