About the RoleThe Frailty Nurse will work closely with the multidisciplinary team to deliver advanced nursing practice and services for older and vulnerable patients. This will include providing support and care to patients with complex frailty needs, working with patients and their families to develop care plans, and promoting health and wellbeing.
Key Responsibilities- Provide proactive care to patients with frailty and long-term conditions
- Work closely with the multidisciplinary team to deliver advanced nursing practice and services
- Support patients and their families to develop care plans and promote health and wellbeing
- Liaise and communicate effectively with the multidisciplinary and wider community teams
- Attend clinical governance and other multi-disciplinary meetings as required
The successful candidate will have current NMC registration and experience of managing long-term conditions. They will also have knowledge of protocols and clinical guidelines, as well as experience of audit and management of long-term conditions. A non-medical prescriber qualification would be desirable.
