We are seeking a dedicated Clinical Nurse Specialist to provide exceptional care and education to our Peritoneal Dialysis patients. This role offers the opportunity to work independently while making a significant impact on patients' lives.
Key Responsibilities:
- Manage a caseload of patients, ensuring their ongoing treatment is effective and on track.
- Conduct scheduled home visits within a designated area, providing personalized care.
- Educate patients on self-administration of dialysis, empowering them to take control of their health.
- Collaborate with healthcare teams to develop comprehensive care plans tailored to each patient's needs.
- Flexibly manage your own schedule while balancing on-call responsibilities.
- Provide support and mentorship to patients, guiding them through their treatment journey.
Qualifications:
- Active Registered Nurse (RN) license in the relevant state.
- A minimum of 12 months of nursing experience is required.
- Strong organizational skills and a passion for patient-centered care.
- Associate's Degree in Nursing (A.D.N) or higher from an accredited institution.
- Proficiency in basic computer applications, including MS Word and Outlook.
