Job Title: Adult's Senior Social Worker

At Randstad Care, we are seeking a dedicated and compassionate Adult's Senior Social Worker to join our team. As a key member of our Older People and Disabilities Team, you will play a vital role in supporting adults with complex needs, particularly older individuals and those with physical disabilities.

Key Responsibilities:

  • Assess the needs of adults with complex needs, including older individuals and those with physical disabilities.
  • Develop and implement personalized care plans to enhance independence, wellbeing, and quality of life.
  • Coordinate access to services that support adults with complex needs.

Requirements:

  • Experience working with adults with complex needs, particularly older individuals and those with physical disabilities.
  • Strong assessment and care planning skills.
  • Ability to work collaboratively with a range of stakeholders, including adults, families, and healthcare professionals.
