About the Role

We are seeking a highly skilled and dedicated Social Worker to join our team at East Riding of Yorkshire Council. As a Social Worker, you will play a vital role in supporting vulnerable adults and their families, providing person-centred assessments, and developing collaborative care and support plans.

Key Responsibilities

  • Undertake comprehensive assessments of adults in need, identifying their strengths and needs, and developing tailored care and support plans.
  • Work collaboratively with individuals, families, and other professionals to achieve positive outcomes and promote independence.
  • Participate in a duty rota, responding to crisis and urgent situations, and providing timely support and interventions.
  • Develop and maintain effective relationships with service users, carers, and other stakeholders, ensuring that their needs are met and their voices are heard.
  • Stay up-to-date with relevant legislation, policies, and procedures, including the Care Act 2014, Mental Capacity Act 2005, and adult safeguarding.
  • Contribute to the development and implementation of policies and procedures, ensuring that they are effective and responsive to the needs of service users.

Requirements

  • Excellent communication and interpersonal skills, with the ability to build rapport with service users, carers, and other professionals.
  • Strong assessment and planning skills, with the ability to identify and address complex needs.
  • Ability to work under pressure, managing competing priorities and deadlines, and maintaining a high level of professionalism.
  • Flexibility and adaptability, with the ability to respond to changing circumstances and priorities.
  • Excellent written and IT skills, with the ability to use relevant systems and software, including the adult database Azeus.
