We are pleased to announce an opportunity for a dedicated and proactive individual to become a part of the dynamic Community Support Services team. This role is crucial in providing assistance to individuals over the age of 18 within their residential settings.
The Community Support Technician team is vital for the effective operation of our services, playing an integral role in managing referral processes and coordinating care for new clients who have recently transitioned from medical facilities, as well as those requiring support for rehabilitation. This position is essential in ensuring the delivery of a high-quality, efficient service that adheres to regulatory standards.
To excel in this role, candidates must demonstrate strong time management skills, organizational abilities, and the capacity to meet deadlines in a fast-paced environment.
A solid educational background is required, along with proficient written and verbal communication skills, as well as a good command of literacy and numeracy.
Key Responsibilities:
- Processing referrals from healthcare facilities and the Contracts and Review Team.
- Assigning care appointments through our electronic scheduling system.
- Engaging and communicating with both internal and external stakeholders.
- Accurately entering data into the internal management system.
- Generating reports and providing feedback, including statistical insights to management within specified timelines.
- Participating in meetings as necessary and contributing to the ongoing enhancement of operational systems.
Proficiency in information and communication technology (ICT) is essential, along with confidence in utilizing various IT tools and software.
