About the Role

We are seeking a motivated and caring Home Care Practitioner to join our Eden Integrated Care Community team. This part-time position (30 hours per week) offers an opportunity to make a real difference in the lives of individuals within this beautiful area.

As a member of our multidisciplinary team, you will work closely with patients in their homes, providing short-term support to maximize independence and enhance quality of life. Your role will involve:

  • Facilitating early discharge from hospital or preventing unnecessary admissions
  • Collaborating with local health and social care teams
  • Providing person-centered care tailored to individual needs
  • Working collaboratively within a friendly and supportive team environment

The ability to work across seven days is essential, with shifts operating between 8 am and 10 pm. Flexibility and a commitment to providing exceptional care are key attributes for this role.
