Job Summary

We are seeking a highly skilled and experienced Case Manager to join our team at Essex County Council. The successful candidate will play a critical role in transforming health and social care services for individuals with learning disabilities and/or autism.

Key Responsibilities
  • Provide specialist advice and professional expertise to health professionals and social workers
  • Demonstrate expert and effective practice in complex situations
  • Work with and support colleagues to mitigate high levels of risk
  • Make independent decisions and troubleshoot to ensure barriers are removed and issues resolved
  • Chair Care and Treatment Reviews for individuals on our Dynamic Support Register
