About the Role

We are seeking a highly skilled and compassionate Senior Healthcare Support Worker to join our Community Nurse teams at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to patients in their own homes or residential care environments.

Key Responsibilities
  • Provide nursing care to patients in the community, working under the direction of the Nurse (Community) and Nurse Specialist (Community)
  • Contribute to the assessment of care needs and the delivery of planned care under delegation and direction of the Registered Nurse
  • Work collaboratively with other members of the multi-disciplinary team, including GPs and Adult Social Care Team
  • Use electronic recording systems to document patient care and maintain accurate records
  • Participate in regular 121s and yearly appraisals to ensure ongoing development and support
Requirements
  • Level 3 Diploma/NVQ 3 in Health and Social Care or equivalent qualification
  • Experience of working with older people in a community setting (desirable)
  • Competency with IT and electronic recording systems
  • Ability to work independently and as part of a team
