Senior Healthcare Support Worker

This is a fantastic opportunity to join our Community Nurse teams as a Senior Healthcare Support Worker. We are based in a rural/coastal area and cover 6 GP practices with 3 Community Nurse teams to support them with a range of care aspects and clinical skills for patients delivered in their own home or residential care environment.
Key Responsibilities- Provide nursing care to patients in the home environment or appropriate care setting.
- Work under the direction of the Nurse (Community) and Nurse Specialist (Community) and Registered Nurse.
- Contribute to the assessment of care needs and the delivery of planned care under delegation and direction of the Registered Nurse.
- Work as part of a team to meet the needs of the service.
- A Level 3 Diploma/NVQ 3 in Health and Social Care or an equivalent qualification.
- Experience of working with older people in a community setting.
- Competence with IT, including electronic recording.
- A comprehensive induction programme.
- Support to complete the National Care Certificate.
