Senior Healthcare Support Worker

We are seeking a highly skilled and compassionate Senior Healthcare Support Worker to join our Community Nursing Team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to patients in their own homes or community settings.
Key Responsibilities- Work as part of a multidisciplinary team to deliver patient-centered care, promoting independence and well-being.
- Assess and identify the needs of patients, developing and implementing personalized care plans.
- Provide support and guidance to patients and their families, promoting health education and self-management.
- Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
- Contribute to the development and implementation of care pathways and protocols, ensuring best practice and evidence-based care.
- Level 3 Diploma/NVQ 3 in Health and Social Care or equivalent qualification.
- Experience of working in a community setting, preferably with older adults.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Competency in IT, including electronic recording systems.
- Ability to work independently and as part of a team, with a flexible approach to working hours and shifts.
- A comprehensive induction program, including support to complete the National Care Certificate.
