Job Title: District Nurse Team Lead

We are seeking a highly skilled and experienced District Nurse Team Lead to join our Community Nursing Teams in Brent. As a key member of our team, you will be responsible for providing clinical leadership and management to our District Nursing team, ensuring the delivery of high-quality care to patients in the community setting.

Key Responsibilities:
  • Provide clinical leadership and management to the District Nursing team, ensuring the delivery of high-quality care to patients in the community setting.
  • Develop and implement effective care plans, working closely with patients, families, and healthcare professionals to achieve optimal outcomes.
  • Collaborate with other healthcare teams to ensure seamless care coordination and effective communication.
  • Conduct comprehensive assessments, identify patient needs, and develop individualized care plans to meet those needs.
  • Provide expert clinical advice and support to staff, promoting evidence-based practice and continuous quality improvement.
  • Work closely with the Clinical Development lead to organize and develop training programs for District Nurses, ensuring they have the skills and knowledge needed to deliver high-quality care.
Requirements:
  • Registered Nurse with a valid NMC registration.
  • Proven experience in clinical leadership and management, preferably in a community nursing setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong clinical skills, with the ability to assess patient needs and develop effective care plans.
  • Ability to work autonomously and as part of a team, with a strong commitment to continuous quality improvement.
