- Develop and implement effective care plans, working closely with patients, families, and healthcare professionals to achieve optimal outcomes.
- Collaborate with other healthcare teams to ensure seamless care coordination and effective communication.
- Conduct comprehensive assessments, identify patient needs, and develop individualized care plans to meet those needs.
- Provide expert clinical advice and support to staff, promoting evidence-based practice and continuous quality improvement.
- Work closely with the Clinical Development lead to organize and develop training programs for District Nurses, ensuring they have the skills and knowledge needed to deliver high-quality care.
- Registered Nurse with a valid NMC registration.
- Proven experience in clinical leadership and management, preferably in a community nursing setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong clinical skills, with the ability to assess patient needs and develop effective care plans.
- Ability to work autonomously and as part of a team, with a strong commitment to continuous quality improvement.
