About the Role

We are seeking a compassionate and enthusiastic District Nurse Team Leader to join our diverse and supportive team. As a key member of our District Nursing service, you will play a vital role in delivering high-quality care to our patients and supporting the development of our staff.

Key Responsibilities
  • Work in partnership with patients and their families to develop and agree tailored health plans to address their needs.
  • Collaborate with other healthcare providers to ensure seamless delivery of services for clients and communities.
  • Lead and manage a team of nurses to deliver high-quality care and support the development of staff.
  • Contribute to the development of our District Nursing service, identifying areas for improvement and implementing changes to enhance patient care.

Person Specification
  • Registered Nurse with current UK NMC registration.
  • Evidence of continuing professional education and training at degree level.
  • Qualification in team/service management or prepared to work towards.
  • Mentor or practice-based learning training.
  • Community or Independent Nurse Prescribing qualification or prepared to work towards.
  • Post-registration qualification in district nursing or equivalent.
Experience
  • Evidence of recent post-registration nursing experience in community nursing.
  • Experience of evidence-based care.
  • Experience of undertaking comprehensive health needs assessments and developing complex care plans.
  • Experience of co-ordinating and delivering care for people with complex, long-term conditions.
Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
