About the Role

We are seeking a highly skilled and experienced District Nurse Team Leader to join our team. As a key member of our community nursing service, you will be responsible for leading a team of nurses and supporting the delivery of high-quality patient care.

Main Responsibilities
  • Lead a team of nurses in the delivery of community nursing services
  • Develop and implement care plans to meet the needs of patients with complex health conditions
  • Work collaboratively with other healthcare professionals to ensure seamless delivery of services
  • Manage and coordinate the work of the team to ensure efficient and effective service delivery
  • Provide guidance and support to team members to enhance their skills and knowledge
Requirements
  • Registered Nurse with current UK NMC registration
  • Evidence of continuing professional education and training at degree level
  • Qualification in team / service management or prepared to work towards
  • Mentor or practice-based learning training
  • Community or Independent Nurse Prescribing qualification or prepared to work towards
  • Post-registration qualification in district nursing or equivalent
