About the Role

We are seeking a highly skilled and experienced District Nurse Team Leader to join our team in Merton Locality. As a key member of our adult services, you will play a vital role in delivering high-quality care to our patients.

Main Responsibilities
  • Work collaboratively with patients and their families to develop and agree tailored health plans
  • Participate in service development activities to improve patient outcomes
  • Work in partnership with other healthcare providers to ensure seamless delivery of services
  • Manage a team within the locality to achieve excellent patient care

Requirements
  • Registered Nurse with current UK NMC registration
  • Evidence of continuing professional education and training at degree level
  • Qualification in team/service management or prepared to work towards
  • Community or Independent Nurse Prescribing qualification or prepared to work towards
  • Post-registration qualification in district nursing or equivalent
Experience
  • Evidence of recent post-registration nursing experience in community nursing
  • Experience of negotiating and liaising within a multi-disciplinary team
  • Experience of working with people with complex needs
  • Experience of co-ordinating and managing a team
  • Experience of teaching others and mentoring students
  • Experience of evidence-based care
  • Experience of undertaking comprehensive health needs assessment and developing complex care plans
  • Experience of co-ordinating and delivering care for people with complex, long-term conditions
