Job Title: District Nurse Team Manager

We are seeking a skilled and experienced District Nurse Team Manager to join our team at Hounslow and Richmond Community NHS Healthcare Trust. As a key member of our community nursing team, you will be responsible for managing a caseload of patients in Hounslow, providing clinical leadership and management to the team, and working in partnership with primary healthcare teams, social services, and other stakeholders to deliver high-quality patient care.

Main Responsibilities:
  • Provide clinical leadership and management to the Hounslow Community Nursing team
  • Manage a caseload of patients in Hounslow, including case management of patients at risk of hospital admissions
  • Work in partnership with primary healthcare teams, social services, and other stakeholders to deliver high-quality patient care
  • Develop and implement plans to improve patient outcomes and reduce hospital admissions
  • Lead by example, demonstrating a commitment to excellence in patient care and teamwork
Requirements:
  • Qualified RGN with current registration with NMC
  • Diploma or BSc (Hons) Community Health (DN)
  • Relevant post-registration study at diploma/degree level and relevant CPD
  • Nurse Prescriber
  • Physical Assessment Skills
Person Specification:
  • Holistic Assessment skills
  • Core clinical skills
  • Ability to work independently and as part of a multi-disciplinary team
  • Ability to lead by example, both clinically and managerially
  • Conflict management and mediation skills
  • Able to demonstrate understanding of Safeguarding Adults & Children
  • Good knowledge of NMC Guidelines and Standards
Experience:
  • Experience in assessing, planning, delivering, and evaluating patients in their own homes
  • Experience in working independently and as part of a team
  • Experience in problem-solving and supporting other team members
  • Experience of multidisciplinary team work
  • Experience of mentoring students
  • Experience of managing resources to stay within the service budget

£51,883 to £58,544 per annum
