Practice Nurse

We are seeking a skilled and dedicated Practice Nurse to join our team at Hillview Family Practice. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.
Key Responsibilities- Act as a key member of our nursing team, providing expert care to patients and supporting the practice's mission to deliver exceptional healthcare services.
- Develop and implement effective care management programs for frail and housebound patients, ensuring their needs are met and they receive the best possible care.
- Assess patient needs, providing evidence-based care and promoting health and wellbeing through health promotion and wellbeing programs.
- Implement and evaluate individual treatment plans for chronic disease patients, promoting health and wellbeing and preventing adverse effects on patients' health.
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide routine nursing care to patients, adhering to clinical-based evidence, NICE, and NSF guidelines.
- Diagnose and manage acute and chronic conditions, prescribing medication as necessary and reviewing patient medications to enhance compliance.
- Provide guidance to patients on the use of prescribed and over-the-counter medications, including side effects and dosages.
- Coordinate the provision of travel medicine services, requesting pathology services, and processing pathology results as required.
- Oversee the provision of chronic disease clinics, delivering patient care as necessary and referring patients to secondary/specialist care as required.
- Maintain accurate clinical records, ensuring read codes are used effectively by all members of the nursing team and ensuring the appropriate maintenance and use of chronic disease registers.
- Develop, implement, embed, and participate in well woman and well man clinics, ensuring compliance with the practice chaperone policy.
- Assist GPs with minor surgery when required, prioritizing health issues and intervening appropriately, and supporting the team in dealing with clinical emergencies.
- Recognize, assess, and refer patients presenting with mental health needs, implementing vaccination programs for adults and children, and supporting patients in the use of their prescribed medicines or over-the-counter medicines.
- Liaise with external services/agencies to ensure patients are supported appropriately, delegating clinical responsibilities appropriately, and supporting the clinical team with all safeguarding matters.
- Understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately, and delivering opportunistic health promotion where appropriate.
- Registered Nurse (Nursing and Midwifery Council)
- Experience of working autonomously, infection prevention and control measures, and chronic disease management
- Ability to work under pressure/in stressful situations, effectively utilize resources, and commit to ongoing professional development
- Postgraduate diploma or degree in Chronic Disease Management, Nurse prescriber, or Family planning qualification
