Independence and Wellbeing Specialist

Position Overview:
We are excited to present an opportunity within the South Rural Integrated Neighbourhood Team (INT) for an Independence and Wellbeing Practitioner. This role is available on a 12-month secondment or fixed-term basis.
Key Responsibilities:
- Collaborate with clients to determine and implement tailored, high-quality outcomes that enhance their independence and overall wellbeing.
- Engage as part of a regional social work unit, working alongside colleagues and professionals from the Health and Voluntary sectors to facilitate seamless integration between health and social care services.
- Effectively communicate Suffolk County Council's vision, objectives, and priorities to clients, partners, and the community, while embodying our organizational values.
- Foster a culture centered on value for money, outcome-oriented customer service, and ongoing improvement.
Essential Qualifications:
- Ability to travel across a broad rural area to assist individuals with Care Act Assessments, evaluate eligibility criteria, conduct care and support reviews, and address safeguarding matters.
- Strong communication skills.
- Experience with LiquidLogic (LAS) is preferred.
- Capability to work independently as well as collaboratively within a team.

About the Team:
The South Rural INT team consists of professionals from Adult Social Care (ASC), Community Nursing, and Community Therapy. Our social work team in ASC supports adults (aged 18+) and provides a comprehensive Adult Social Work and Occupational Therapy service. The South Rural team is dedicated to assisting adults within the 'South Rural' geographical area, addressing care and support needs that extend beyond the short-term services provided by the Independence and Wellbeing Service (IWS) and REACT.
