About the Role

We are seeking a dynamic and motivated Band 5 Public Health Staff Nurse to join our team in the newly integrated 0-19 Children's and Young People Public Health Service. As a key member of our team, you will work closely with children, young people, families, primary care, schools, and statutory and voluntary services to deliver health-enhancing activities and improve health outcomes.

Main Responsibilities
  • Contribute to the safe and effective delivery of the 0-19 service within your caseload and as delegated by the Specialist Community Public Health Nurse.
  • Work in partnership with children, young people, families, primary care, schools, and statutory and voluntary services to facilitate health-enhancing activities and improve health outcomes.
  • Deliver services to children and families within homes and community settings.
  • Assess, plan, implement, and evaluate family-focused care under the delegation of a SCPHN Health Visitor/School Health Nurse and work in partnership with other agencies.
  • Undertake Healthy Child Programme developmental and transitional assessments and planning, delivering and evaluating public health initiatives through community-based health promotion activities.
