Job Title: Pediatric Community Nurse - Complex Care Specialist
Job Summary:
We are seeking an experienced Pediatric Community Nurse to join our team at Thornbury Community Services. As a Pediatric Community Nurse, you will be responsible for delivering high-quality care and support to young people with complex needs in a community-based setting.
Key Responsibilities:
- Deliver bespoke care and support to young people with complex needs
- Work collaboratively with the client's family and other healthcare professionals to achieve personal goals and improve quality of life
- Conduct regular assessments and reviews to ensure the client's needs are met
- Develop and implement individualized care plans to promote independence and well-being
Requirements:
- Minimum of 6 months' experience in pediatric nursing
- Recent experience in complex care nursing, including experience with nasopharyngeal suction, mechanical cough assist, and gastrostomy care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
