Job Summary
We are seeking a qualified practitioner to join our Learning Disability Team as a Community Practitioner. The successful candidate will work within a multi-disciplinary service supporting individuals with learning disabilities and care needs.
Key Responsibilities
- Complete Community Deprivation of Liberty (DOL) applications to the Court of Protection (COP)
- Manage an allocated caseload of clients from across the LD service, living in community supported living placements or in their own/family home
- Work within expected time frames, understanding the legal framework and building effective working relationships with LCC Legal services
- Undertake non-dedicated DoLS duties as a Qualified Practitioner to support the Service, such as undertaking new Adult Care assessments or reviews or responding to emergencies or safeguarding concerns
Requirements
- Hold a Social Work or Nurse qualification and be registered with SWE/NMC
- Have creative thinking and problem-solving skills
- Be able to work flexibly and respond to the needs of the Service
