About the Role

We are seeking a highly motivated and experienced Community Public Health Nurse to join our team at Oxford Health NHS Foundation Trust. As a key member of our 0-19 Children and Young People Public Health Service, you will play a vital role in delivering high-quality health care to children, young people, and families in Oxfordshire.

Key Responsibilities
  • Contribute to the safe and effective delivery of the 0-19 service, working in partnership with children, young people, families, primary care, schools, and statutory and voluntary services.
  • Deliver services to children and families within homes and community settings, including assessing, planning, implementing, and evaluating family-focused care.
  • Undertake Healthy Child Programme developmental and transitional assessments and planning, delivering and evaluating public health initiatives through community-based health promotion activities.
  • Work with partnership agencies and develop, participate, and maintain interagency networks.
