Job Summary

We are seeking an experienced and motivated Senior Clinical Pharmacist to join our team at East London NHS Foundation Trust. This is an exciting opportunity to work as part of a multi-disciplinary team, providing clinical pharmacy services to Cauldwell Medical Centre and contributing to the improvement of patient care.

Main Responsibilities
  • Work collaboratively with healthcare professionals to provide high-quality clinical pharmacy services.
  • Undertake clinical medication reviews to proactively manage patients with complex poly-pharmacy.
  • Provide primary support to general practice and GPSU staff with regards to prescription and medication queries.
  • Lead the development and implementation of medicines optimisation initiatives.
  • Contribute to the quality and outcomes framework and enhanced services.
About the Role

This role is pivotal to improving the quality of care and operational efficiencies. We are looking for an individual who is passionate about delivering excellent service within general practice and has the skills and experience to make a positive difference to patients' lives.

Requirements
  • Professional registration with the General Pharmaceutical Council.
  • Masters degree in pharmacy or equivalent.
  • Experience of working as a qualified, registered pharmacist in primary care, community or hospital pharmacy.
  • Independent prescriber.
  • Completed the Primary Care Pharmacy Education Pathway of the CPPE.
  • Awareness of general practice and the agenda for medicines optimisation.
