Job Summary
We are seeking a highly skilled and compassionate Support Worker to join our Adult Enablement Unit team. As a Support Worker, you will play a vital role in providing practical and emotional support to adults with complex mental health and learning disabilities.
Key Responsibilities
- Support clients to access and engage in their local community, promoting social inclusion and independence.
- Accompany clients to appointments, walks, and activity programs, fostering a sense of belonging and connection.
- Administer medication and support medication management, ensuring the well-being and safety of clients.
- Assist clients with budgeting, enhancing their financial literacy and independence.
- Develop and implement personalized care plans, promoting client-centered care and empowerment.
- Provide evening meals from a set menu, promoting healthy eating and nutrition.
- Crisis planning and care planning, ensuring clients receive timely and effective support.
Requirements
