Job Overview

Homerton Healthcare NHS Foundation Trust is seeking a highly skilled and dedicated Children's Community Health Specialist to join our Community Children's Nursing team. As a key member of our team, you will play a vital role in providing high-quality nursing care to children in the community, working closely with parents and carers to ensure the best possible outcomes for our patients.

Main Duties and Responsibilities
  • Provide specialist nursing care to children in the community, working autonomously and as part of a team to deliver high-quality care.
  • Develop and implement individualized care plans, working closely with parents and carers to ensure the best possible outcomes for our patients.
  • Facilitate the development of the extended role of the Community Children's Nursing team, teaching, supporting, and acting as a resource for all members of the team.
  • Work in close partnership with parents and carers to ensure they are confident and competent in the care of the sick child at home.
  • Manage client caseloads and have the ability to work autonomously, delegating work and supporting junior staff members as needed.
  • Be responsible for the provision of assessment, delivery, and evaluation of planned programmes of care.
  • Manage your own time and priorities in collaboration with other members of the Community Children's Nursing Team.

Person Specification

We are looking for a highly skilled and dedicated Children's Community Health Specialist who possesses the following essential criteria:

  • Register Child Teaching/Mentoring 998 or equivalent.
  • Ability to manage a caseload of children with varying health needs.
  • Ability to communicate complex and highly sensitive information.
  • Ability to link with the acute units as necessary to ensure the smooth transfer from the ward into the home setting.
  • Ability to work within a team and contribute to the smooth running of the team.

We also welcome applications from individuals who possess desirable criteria, including Community Specialist Practitioner and experience of nursing children who have required ventilation.
